Geriatric Care Management

Geriatric Care Managers (GCM) are health and human service specialists. They are trained in a dynamic background not limited to gerontology, nursing, counseling and social work. GCMs aid in the process of caring for an elder and/or disabled loved one to improve their quality of life and achieve their maximum functional potential. A personal care manager will evaluate the individual with a care-planning assessment and identify problem areas. GCMs have extensive knowledge on the quality, cost and services in your community and will provide individually fit solutions.
